Community Based Support in Devon

Well UK is a lead provider on Devon County Council's (DCC) 'Framework' for 'Social Care and Support Services that Enable People to Live in the Community'.

Well UK leads a consortium of 15 and their fields of work and specialisms are described below. Our partners are all charities or 'not for profit' organisations and have been carefully selected for their expertise in their areas of specialisation and for their reputation in the community.

Devon (excluding Torbay and Plymouth) has been divided into 8 geographic zones by DCC and the Well UK consortium is able to offer services in all. The zones are as follows:

Age UK Exeter bases its work on the principle that everyone is different and deserves the best service. The team of 100+ staff and over 200 volunteers therefore work hard to respond to each person as the individual they are by being flexible, valuing diversity and fighting against ageism. Support includes:

-Supporting Independent living e.g. daily living skills
-Social Opportunities
-1:1 advice e.g. managing finances, budgeting

Age UK Tiverton Cullompton & District3OP

Its aim is to improve the quality of life for older people and their carers. Age UK Tiverton Cullompton and District seeks to ensure that older people maintain their self-respect and independence and enjoy freedom of choice and equal opportunities. Support includes:
-1:1 support including advice, help with daily living and enabling people access services
-Day Service Activity Centre
-Basic IT support
-Foot-care
-Lunch Club
-Information and Advice
-Advocacy e.g. accessing benefits, support after leaving hospital

CEDA34LD PD

CEDA is a registered charity, based in a purpose adapted building on the outskirts of Exeter providing support services to disabled people with a focus on independence and empowerment. CEDA prides itself on responding to service users' needs and finding imaginative and creative ways to work with disabled people. Support offered includes:
-Enabling - helping clients to make links with community organisations and groups
-10 - 3 A range of activities, chosen by the service users- social & learning e.g. IT, literacy, life skills, Tai Chi etc.
-Short Breaks - arranging bespoke short holidays, going to destinations of their choice, when they want to go with staff they feel comfortable with
-Person Centred Planning - helping clients make plans for their lives- what's important, what they want to change and steps forward.

Devon Link-Up45LD

Devon Link Up is a user-led organisation for people with learning disabilities. That means that people with learning disabilities run Devon Link-Up and make sure that it meets the needs of all its users. Devon link Up believe that everyone has the right to a voice so they work together to make sure voices are heard in Devon. We do this through the Parliament, Speak Up Groups and support services:
-Support to be more independent
-Support to access volunteering opportunities
-Support to find work
-Help to find social opportunities
-Healthy eating programmes
-Developing confidence
-Budgeting
-Managing the transition into adulthood
-Support at court of with the solicitor
-Speaking-up groups and The Learning Disability Parliaments

Dimensions support people with learning disabilities and people with autism. Its aim is to enable people to be part of the community and make their own choices and decisions about their life. The support offered includes:
-Support for people living in their own homes or in shared housing
-Support for people to find and maintain suitable accommodation
-Support advisors to help individuals and their families with a personal budget
-Transition to adulthood
-Opportunities for learning, employment and participating in the community
-Day services and outreach

Exeter Community Initiatives4LD MH

Exeter Community Initiatives is a local charity, passionate about helping people by working with them to run projects that meet their needs. We believe anyone experiencing vulnerability in their life can fulfil their potential given the right opportunities and support.
-Support to access community based health and well-being  services
-Encouraging Healthy lifestyles
-Develop the confidence to become involved in wider community
-Support to meet spiritual and faith needs
-Support to develop independent living skills including finding and setting up a home, personal finances and debt management

Living Options Devon exists to ensure people with physical and/or sensory disabilities and Deaf people with sign language can make an active and equal contribution in society. All of its work with local disabled and Deaf people is about identifying needs, challenging attitudes and achieving positive change. Support offered includes:
-Community support- enabling people with physical and sensory disabilities to take part in community and social activities.
-Personal Development: Training, mentoring, coaching and other support to develop a range of skills and increase confidence
-Positive Devon Living and Living Options Membership association
-A range of peer support and volunteering opportunities for people with disabilities

Mencap is a leading voice of learning disability. Everything they do is about valuing and supporting people with a learning disability, and their families and carers. Services include:
-Prevention and Intervention Services
-Support for families and individuals
-Outreach to isolated people
-Transition to adulthood
-Finding a Home
-Supported employments
-Respite Care
-Day opportunities

Parkview Society3456LD MH PD OP**

The Parkview Society, through a caring, motivated and appropriately trained staff team, tailors its services to suit the needs of individuals with mental health issues, learning difficulties, housing problems and other vulnerabilities. Support includes:
-Accessing Services - Support to arrange and transport to health, professional and benefits services
-Social Recreational and supporting physical and emotional Well-being
-Personal Organisation support - home management, personal hygiene, access to shopping, neighbour relations.  Clean up service
-Sitting and Escort Services

Upstream Healthy Living Centre35OP

Upstream reaches out into local communities to help people who are mentally, physically or socially isolated to improve their own health, well-being and quality of life and to prolong their independence. Upstream offers:
-One-to-one enabling
-Support for clients to find and access existing services & organisations
-Healthy Living - provision of craft and exercise groups

Westbank Community and Care678OP

Westbank provides care and promotes health to communities across Devon. Its mission is 'Providing Care, Improving Health'.  Amongst its core values are to inspire and enable people to unlock their true potential and that of others. Westbank has 100 staff supported by 400 volunteers and its services for older people include:
-Day Care facility for older people
-Volunteering opportunities - Supported volunteering available across a wide range of functions at Westbank and in the community
-1:1 support for older isolated people
-Preventative and rehabilitation classes - e.g. Falls Prevention Programme, Cardiac Rehabilitation Programme
-Exercise - Public gym, health walks, various fitness classes e.g. yoga, gym circuit training for different levels and groups.
-Social Clubs across Zones 6,7 & 8 e.g. Weekly Gents social club for men who are either on their own or who require the company of males, Ladies Friendship Group, a social opportunity for women.

Additionally we are also working with Headway (Devon), the specialist provider of support for people with aquired brain injury and MIND in Exeter & East Devon - the mental health charity.

Kazlum Support3,4MH, LD

Kazlum is a small family run business supporting people with mental health problems and those with learning disabilities. Based in Lapford, Mid Devon and also working in Exeter Kazlum Support provides:
- Staffing to meet the needs of services users 365 days a year
- 24 hour on call support service
- Support from a few hours a week, up to 24 hours a day, depending on the assessed needs of the service user
- The development of future person centred plans
- A service based around individuals assessed needs and aspirations, in line with personal plans.
- The maintenance of links with families, as appropriate
- Support with development of social and personal skills
- The review of services on a regular basis
- The provision of information and advice
- The provision of other domestic or community based supports as required.

Support from a well-trained team of support staff who, wherever possible, will be chosen by the person to be supported and their friends and or family.
